Parents and Guardians:
On August 21, 2017, the CDSD school board approved a new over the counter medication policy which will allow certain non-prescription medications to be dispensed to district students at the school nurse’s discretion. If you would like your child to be able to receive Ibuprofen, Tylenol or Tums, this form must be signed and returned to the nurse’s office prior to administration of these medications. This form will only need signed once and will remain on file as long as your son/daughter is enrolled in the district. Once you’ve allowed your son/daughter to receive these medications, you may alter your permission by contacting the school nurse.
