Required Medical Examinations

A medical examination is required for all pupils upon original entry to school (kindergarten or first grade), sixth grade, and eleventh grade.  Parents are encouraged to have the physical done by their family physician because he/she is familiar with the pupil’s medical history. Private physician report forms are provided at the appropriate times.

Pupils not examined by the family physician will be scheduled for examination by the school physician. Times for these examinations will be scheduled in advance and parents are welcome to be present during the examination.

Physical problems discovered or suspected by the examining physician or a private physician are reported to the parents. Fees incurred for examinations by private physicians are the responsibility of the student.

School physicians are provided for examination of candidates for athletic squads. According to school district policy, each pupil must have a physical examination by the school physician or a private physician for each sport in which he/she wishes to participate. This physical examination must be done before the student may attend practice. Fees incurred for examinations by private physicians are the responsibility of the athlete.
